As of May 8, 2015, all Social Adult Day Care Centers (SADCs) in New York State are required to complete certification each year by June 30th. Once your certification is complete, you must attach the approval and submit it along with the information below so we can record that you have met this requirement. If you have not yet completed certification, or would like more information on this requirement, please visit the New York State Office of Medicaid Inspector General website.
2016 Annual Site Visit Requirements:
In accordance with Section VII.C.2.b of the MLTC contract with the State of New York, Healthfirst is required to site visit all contracted Social Adult Day Care (SADC) providers “Prior to entering into contract with a provider of Social Day Care services, and on an annual basis thereafter. Contractors are required to conduct a site visit of each such provider in their network to review and assure compliance with:
i. 9 NYCRR 6654.20,
ii. The terms of the contract between the provider and Contractor, and
iii. All other standards required by law or regulation for the operation of said provider, including but not limited to laws, codes, and regulations regarding the facility’ structure, labor requirements, and food quality.”
In adherence, Healthfirst is requiring all participating SADCs to submit a signed attestation (attached) and a copy of the most recent “SADS Self-Monitoring Tool” submitted to the NY State Department of Health. All documents should be submitted by visiting www.hfwageparity.org and uploading your (1) signed attestation and (2) “SADS Self-Monitoring Tool.”
